Hormone Therapy Options: Understanding Your Choices for Better Health

We Use Bioidentical Hormones
At Align Healing Center, we work with bioidentical hormones. These are structurally identical to what your body already produces, so they are recognized and processed the way your own hormones would be. We also do not use oral estrogen, and this comes down to one of the biggest concerns with hormone therapy: blood clots. When estrogen is taken by mouth, it passes through the liver before reaching the bloodstream, and this first pass raises certain clotting factors. Delivering estrogen through the skin or as a pellet bypasses the first pass through the liver entirely, which is associated with a lower risk of clotting. This is a major reason we choose these routes for our female patients, so you can feel supported without carrying that added risk.
Much of the fear around hormone therapy traces back to a large study from the early 2000s linking estrogen to higher clot and health risks. What often gets left out is that the study used oral, synthetic hormones in an older population, not bioidentical hormones delivered through the skin. Later research shows that route of delivery makes a real difference in clot risk, which is exactly why we are intentional about the hormones and methods we choose for you.
Finding the Delivery Method That Fits You
There is no single right way to take hormones. The best method often comes down to your daily routine, your comfort level, and honestly, your personality. We take time to get to know each patient so we can match the delivery method to the person, not the other way around.
Creams and gels are applied daily and let us fine-tune your dose in small steps. These suit patients who like being closely involved in their routine and want flexibility while we learn how your body responds.
Suppositories are a preferred option for many patients, especially for estrogen. They are absorbed easily, skip the mess of a daily cream, and offer a simple alternative for patients who want something straightforward to work into their routine.
Troches are small lozenges dissolved in the mouth, and we use them mainly for progesterone. They are absorbed directly into the bloodstream through tissue in the mouth, which offers better absorption than a progesterone pill.
Pellets are tiny implants placed under the skin, usually in the hip, releasing hormones steadily over several months. This suits patients with busy, unpredictable schedules who would rather not think about a daily or weekly routine. Once placed, the dose stays steady until the next round, so we make sure your starting labs are dialed in.
Injections are given on a set schedule, often weekly or biweekly. Patients who like structure tend to appreciate this option, and it gives good dosing control.
